Thyroid, diabetes, etc, menopause, depression... all sorts of things..
The person inside is still the same, my grandmother was very overweight, I never met her but she had a Thyroid and Diabetes condition, this was in the late 1800's and there was no assistance medically in those days.
She died of a brain anrysm.
Anyone of us could end up at 60 and have the same medical condition, so there you go.

that lady looks like a women who came in one of the episodes of Ripley's believe it or not  ....she was the women who turned herself into a cat!!
